Based on the findings of an onsite unannounced state re-licensure survey conducted on January 3, 2024, Best In Service Homecare, Llc, was found to be in compliance with the requirements of 28 Pa. Code, Health Facilities, Part IV, Chapter 51, Subpart A.

Based on the findings of an onsite unannounced home care agency state re-licensure survey conducted on January 3, 2024, Best In Service Homecare, Llc, was found not to be in compliance with the requirements of 28 Pa. Code, Health Facilities, Part IV, Chapter 611, Subpart H. Home Care Agencies and Home Care Registries.

611.56(a) LICENSURE
Health Screening

Name - Component - 00
(a) A home care agency or home care registry shall insure that each direct care worker and other office staff or contractors with direct consumer contact, prior to consumer contact, provide documentation that the individual has been screened for and is free from active mycobacterium tuberculosis.

Observations:



Based on review of personnel files (PF) and interview with the agency administrator, it was determined the agency failed to ensure direct care workers were screened for and were free from active mycobacterium tuberculosis (TB) prior to assignment with consumers in three (3) of ten (10) files reviewed. (PFs # 3, # 4 and # 6)


Findings include:

The CDC guidelines state that all Health Care Workers (HCW) should receive baseline tuberculosis screening upon hire, using a two-step tuberculin skin test (TST) of a single blood assay for tuberculosis (TB) to test for infection with tuberculosis. After baseline testing for infection with tuberculosis, HCWs should receive TB screen annually. HCWs with a baseline positive test for tuberculosis infections should receive one chest radiograph result to exclude tuberculosis disease. Review of personnel files (PF) conducted on 1/3/24 from 12:10 PM-1:45 PM revealed the following:


PF #3, (DOH): 10/5/23, the file contained documentation of only one step TST on hire. TST planted on 2/10/23 and read as negative on 2/12/23.

PF #4, (DOH): 12/20/23, the file contained no documentation of a two-step TST on hire.

PF #6, (DOH): 9/9/23, the file contained no documentation of a two-step TST on hire.




An interview with the agency administrator conducted on 1/3/24 at 2:10 PM confirmed the above findings.






Plan of Correction:

Under the direction of the Agency Human Resources Manager, the agency will refer the following employees for a two-step TST or T-spot (TB blood test);

Pf#3 (DOH) 10/05/2023, PF# 4 (DOH) 12/20/2023 and PF #6 (DOH) 09/09/2023) whose screening results were found to be missing from their files. The result of the screening will be documented in each employee's file once they are obtained.

Furthermore, under the direction of the agency HR Manager, all active employee files will be audited to ensure that TB screening results are documented in each employee's file. This will be completed by 03/02/2024.

(c) A direct care worker employed by a home care agency or rostered by the home care registry on December 12, 2009, shall achieve compliance with the health evaluation requirements imposed by this chapter by September 1, 2010.

Review of personnel files (PF) conducted on 1/3/24 from 12:10 PM-1:45 PM revealed the following:



PF #2, (DOH): 11/18/22, the file did not contain documentation of annual TB screening and education.

PF #9, (DOH): 11/12/22, the file did not contain documentation of annual TB screening and education.


An interview with the agency administrator conducted on 1/3/24 at 2:10 PM confirmed the above findings.






Plan of Correction:

Under the direction of the agency Human Resources Manager, Annual TB screening and education will be provided to the following employees and will be documented in their file as evidence showing that they received the annual TB education.

Furthermore, to ensure that there is no other missing annual TB education, under the direction of the HR Manager, all active employee files will be audited. Any employee found to have missing documentation of annual TB education will be provided and proof stating that they received the education will be documented in the employee's file. This will be completed by 03/02/2024.
